Storage and Distribution of an ADRT
It is advised 
  • The person should keep their own copy of the ADRT and ensure that its existence is readily identified e.g. with a Medic Alert Bracelet, prominent place in the home.
  • The person should be encouraged to give his or her own General Practitioner and other responsible Health and Social Care Professionals providers a copy of the ADRT. The existence of the ADRT should be recorded on paper and electronic records.
Summary list of suggested key people, agencies / organisations who need to be informed of the existence of an ADRT are:
  1. Person /patient
  2. Relative / Carer
  3. Key Worker (health, mental health, social services etc)
  4. General Practitioner
  5. Care Home (if the person/patient is a resident)
  6. Ambulance Services (please see links page for further information or go to http://www.emas.nhs.uk)
  7. Out of Hours Services including NHS Direct
  8. Local Hospital (especially if a known / current patient)
Some people may wish to discuss this with a solicitor and ask them to store a copy of their ADRT but it is not essential.
