they may need the support of professionals with the process.
The Key Worker may be the most likely professional to do
this. |

Worker Role |
Definition: “A
named professional who is ‘best placed’ to
ensure the person receives co-ordinated, holistic and timely
end of life care”
This key worker can be one of several
different people and be in different health care settings,
often depending on the nature of the patient’s
illness, the context of their continuing and proposed
care needs and the competences of those health and social
professionals involved. Professional groups in the community
may include District Nurses, Community Matrons or Community
Specialist Nurses. Within secondary care this may be
a Clinical Nurse Specialist within a specialist Multidisciplinary
Team (e.g Breast Cancer Nurse).
If the person does not have a Key Worker, the following
Health and Social Care Professionals may support the
person in the process of making an ADRT: |

- Patient Advice and Liaison Services
- Independent / Voluntary Sector e.g. Age
Concern
