Consider this as part of your normal holistic care you offer people. Often an ADRT is part of an Advance Care Plan. You might be supporting this patient in the role of the “Key Worker”.
What is Advance Care Planning?
Advanced Care Planning
It is essential to consider this when a sign is recognised that the patient’s condition has got much worse or trigger has been identified. Patients and carers often are aware of their physical changes but have not been given the opportunity to discuss this with professionals. A proactive approach sensitive to the expectations of the patient can be taken.

When formulating an Advance Care Plan the process involves an element of discussion and review between a patient, their relatives / carers and clinicians. The purpose is to:
  • explore their concerns.
  • their understanding of their illness and prognosis.
  • the importance of values or personal goals for care.
  • particular preferences for the types of care or treatment that may become necessary in the future.
