family, health and social care professionals, organisations. Keeping a record of who has the document. |
| 3.
Review |
as necessary,
as decisions can change. Keeping everyone informed |
| 4.
Withdraw |
these
decisions must clearly be withdrawn, or altered, documented
and inform everyone of the withdrawal of the ADRT. |

| Are
there any other people / professionals to consider when dealing
with a person who has lost capacity? |
| There
are specific relationships of those people who may or have
lost capacity to other Decision Making mechanisms e.g. Lasting
Power of Attorney, Court Appointed Deputy, Independent Mental
Capacity Advocate(see glossary)WHICH PAGE IS IT? and responsible senior health
and social care professionals / organisations. The
responsibility of these people and bodies is further explained
in the ADRT Specialist Guidance. There is also much more
