When patients are entering the end of life care pathway it is important to consider the following nationally accepted documents:

The aim is to improve palliative care provided by the whole primary care team by optimising continuity of care, teamwork, advanced planning (including out of hours), symptom control and patient carer and staff support

Liverpool Care Pathway for the Dying Patient (LCP) www.mcpcil.org.uk
The LCP empowers health and social care professionals to deliver high quality proactive care to dying patients and their relatives regardless of diagnosis.
LCP is often used in the final phase, often days to last few weeks.

Advance Care Planning: A Guide for Health and Social Care Staff (2008)
This document highlights the key issues and challenges of incorporating ACP into routine patient care. It contains useful information on the key principles, definitions and related terms of ACP. It also indicates how ACP links to the Mental Capacity Act 2005.

Patients maybe eligible to receive support from NHS Continuing Care. There are different levels which apply. In the last weeks patients may need and receive full nursing care at home or preferred place of care. It is important to contact the specialist determining /assessing officer for your area.
East Midlands Ambulance Service - www.emas.nhs.uk
If you live in the East Midlands (UK) area, you can now register an end of life care decision with East Midlands Ambulance Service by posting it to EMAS Clinical Office - Alferton, Salcombe Court, Meadow Lane, Industrial estate, Alferton Derbs DE55 7RG or fax to 01773 522. 656. Please see their website for more information about this registration process. For non East Midlands (UK) residents please contact your local health care services.
